St Helens & Knowsley Teaching Hospitals - Integrated Community Facility

St Helens and Knowsley Hospitals NHS Trust is implementing an integrated community facility for the residents of Knowsley. The service will be a Nurse Led Transitional Care Unit with a model of care which is based around Knowsley residents who are “medically optimised” and will be provided via an existing Nursing/Residential Home in Knowsley. Some consultant cover is proposed as is ring-fenced social work provision and day to day medical support will be provided by local GPs.

The primary objective is to support the smooth transition of patients who are medically optimised but are unable to be discharged directly back into the community as they are awaiting packages of care, assessments, nursing home of choice or adaptations.

The intention is for St Helens and Knowsley Hospitals NHS Trust to lead on the development of the facility in partnership with Knowsley CCG and the Local Authority.

The Trust is seeking expressions of interest from organisations with appropriate accommodation within 2 miles of Huyton Village (L36 6GA) which can provide a 14 bed facility including catering, estates and facilities services.
