NHS Hastings and Rother CCG is seeking to commission a service to train general practices in care planning for patients with long term conditions (LTCs) with an initial focus on diabetes.

The objective of the service is to:

  • Provide practice staff with personalised care planning approach training
  • Ensure the development of motivational interviewing skills
  • Ensure a full understanding of the care planning ethos and patient empowerment.

The Service is outlined below and more fully described in the specification included in the attached documents.

The service must :

  • Deliver a training package to over 40 staff in Hastings and Rother, involving a mixture of GPs and nurses
  • Provide consultancy and support to the CCG on the implementation of a consistent care planning approach
  • Produce materials to support people with long term conditions to be suitably prepared for care planning such as leaflets, posters, letter templates and care planning materials.

The contract will be for an initial one year term. The contract value covers the whole initial term.

The Commissioner is NHS Hastings and Rother CCG. The Commissioner is supported in this project by NHS South East CSU (SECSU) and its procurement partner NHS Commercial Solutions (NHSCS), which is managing this procurement.
