description

Preston City Council (PCC) with partner agencies is looking to develop a new way of working that encompasses the Making Every Adult Matter (MEAM) approach. This approach has been developed by a coalition of Charities Homeless Link, Clinks and Mind. The Council and its partners are looking to develop a co-designed service that is based on assertive outreach with a case management system to assist vulnerable adults who have complex needs to access the services they need in a more co-ordinated way.

This system will put the adult at the heart of the design and will assist in ensuring that mainstream and commissioned services are working in the best way possible to avoid duplication and address the individual's specific needs.

The provider must be able to work flexibly in the development of the MEAM approach including committing to engaging and providing data for a national evaluation which will evidence the impact of the work locally as well as nationally
