Following an external review of the existing services at Granville Court in Hornsea and Millside in Driffield, a service outline has been produced describing the person-centred service outcomes required from a specialist care home for people with profound and multiple learning disabilities and complex physical needs. East Riding of Yorkshire Council’s existing generic service specification for residential care homes is included for information as an indicative guide, but it is our intention for a new service specification to be agreed with the future provider of this service, supplemented by individual care contracts for the residents.
The service outline describes the service currently provided and an indicative service model for future development at a new site, to be identified and agreed by commissioners and the provider.
Before we begin with procurement we would like to better understand the provider market, the perceived deliverability of the current and future specification locally, and what options are available to develop the service in a new purpose built environment in a new location.
Aims of the service:
- Provide a high quality person-centred service to individuals with profound and multiple learning disabilities and complex physical needs which can be best met in a registered care home setting
- Provide advice, support and short breaks as required for individuals with similar needs who live with family carers
- Improve outcomes and quality of life for those individuals requiring care in this environment
- Maximise value for money
Objectives
| • | To sustain the progress that has been made over the past year in improving the quality and safety of care provided for this vulnerable client group |
| • | To increase opportunities for on and off-site activities for the residents in order to give the best possible quality of life |
| • | To integrate health and social care staff in the current service |
| • | To ensure the right staffing model to provide safe, high quality, person-centred care, with flexibility built in to ensure changing needs can be met and to support planned activities |
| • | To agree with residents and their families or carers as well as commissioners a new service model for development |
| • | To work with residents and their families or carers as well as commissioners to identify potential sites for relocation of the service, with good public transport access, good access to the local community for residents and families, centrally located for reasonable proximity to all parts of East Riding of Yorkshire |
| • | To work with residents and their families or carers as well as commissioners to plan and design a new purpose built facility |
| • | To develop the chosen site and manage the transfer of existing residents to either the new site or an alternative service. |
