NHS Arden & Greater East Midlands Commissioning Support Unit (NHS Arden & GEM CSU) on behalf of NHS North Derbyshire Clinical Commissioning Group (NDCCG), acting as Lead Commissioner on behalf of the Derbyshire Clinical Commissioning Groups (including Erewash CCG, Hardwick CCG and Southern Derbyshire CCG), are offering the opportunity for interested and suitably qualified providers to apply for Grant funding to deliver the priorities of Voluntary and Community Sector Commissioning Grant funded services across Derbyshire.

The voluntary sector is diverse and includes organisations of a variety of sizes, roles and structures. These voluntary sector organisations make a unique contribution to health and wellbeing, reaching people and tackling challenges.

The Commissioners have shown their commitment to working with voluntary sector organisations, and have funded many, over a number of years. In February 2014, the Derbyshire Health and Wellbeing Board, with representation from all the Clinical Commissioning Groups, Derbyshire County Council, the Voluntary sector and other partner organisations, agreed to begin a project to review the effectiveness of the funded voluntary sector. As a result of this work, the Commissioners are re-commissioning services in order to meet the current health and wellbeing priorities, and deliver effective, efficient and sustainable services that improve the Health and Wellbeing of people in Derbyshire.

Pressures faced by the NHS relating to an ageing population and people with more complex needs have increased therefore all services commissioned need to demonstrate an effective use of the limited budget. Commissioners are increasingly focussed on commissioning preventative services to enable people to live independently and avoid crises that may result in admission to hospital or to a residential care home.

This opportunity is seeking applications for the identified priorities listed below:

North Derbyshire CCG and Hardwick CCG Priority –

  • Reducing Social Isolation

Southern Derbyshire CCG and Erewash CCG Priorities-

Early intervention, self-help and prevention
Prevention of hospitalisation and support for discharge
Reducing Social Isolation
