Surrey County Council are seeking a Life After Stroke Support Service to be provided to

  • oPeople who are residents of Surrey who have had a stroke, or are affected by stroke.
  • oCarers and families of people who have had a stroke.

We require the following:

Life After Stroke Support Workers who:

  • oWork on a locality basis (aligned to the 5 CCG and SCC areas) across the whole of Surrey; and who
  • oEach manage a caseload, and undertake a combination of in-reach into hospitals to support early and effective discharge; and
  • oOnce the person returns home, visits post discharge to prevent, delay and reduce further deterioration in health and wellbeing and avoid reliance on more costly interventions, avoid readmission to hospital, or avoid admission into residential or nursing care.

Additionally we will be looking for there to be a provision of information and support services
