Enhanced Menopause Support (Havant)

The aim of this project is to improve the experience of menopause and the future health of women in the relevant age group, by offering a targeted, menopause-focused comprehensive health check, wellbeing consultation and onward referral to eligible women. This is a three year pilot project based in the target area of Havant, which has been identified as a priority area for the Hampshire County Council 'Your Future, Your Health' project.

The PCN will be supported to offer women a by-invitation face to face consultation and assessment as follows:

The service will be delivered over six sessions:

Session one: 10 mins appointment for screening bloods as baseline NHS Health Check bloods + vitamin D, thyroid function tests, full blood count and haematinics

Session two: 60 mins menopause health and wellbeing coach face to face appointment. Health check and holistic review to include:

Measurements and risk scoring as standard NHS HC template

Lifestyle questionnaire

Menopause symptoms and bleeding questionnaire

PHQ9 and GAD

Dietary questionnaire

Social and work situation

Expectations questionnaire

Session three: 30 minute appointment HRT Advanced Nurse Practitioner HRT discussion and plan.

Session four: Personal care planning prescription for health and wellbeing and referral to any additional pathways of care already established e.g fitness classes. Following the MECC criteria the sessions will also ensure any new health issues or other concerns are planned for and signposted.

Session five: 3 month HRT review with advanced nurse practitioner.

Session six: 12 month review, evaluation and feedback
