description
The service provides intensive, accommodation-based, support for 12 vulnerable young women and their children (up to age 2), for a period of between six and 18 months. The service is intended to support young women with a variety of needs including those who may be considered to have complex needs, to enable them to live independently in the community. This may include individuals who may have chaotic lifestyles, a history of offending, substance misuse problems, and mental health problems. The mothers themselves may be care leavers. Overall aim(s) of the service Assess the needs of individuals and provide a planned programme of support, agreed by social care and planned with the service user and partner agencies, which meets the holistic needs of the individual Enable service users to develop key independent living skills and self-confidence to enable them to live independently Enable individuals to successfully maintain their tenancy Support access to training and work opportunities where appropriate Facilitate access to other services in particular health and social care related services, welfare and benefits services, education, training, volunteering and employment Dependent on need, facilitate access to rehabilitation/specialist services, liaising effectively with statutory and voluntary agencies on a collaborative basis. Develop a clear process for planning transition into the community with a comprehensive multi-professional package of support and supervision.
