NEL, on behalf of Southwark CCG, is seeking a supplier that has the right expertise, experience, and innovative mind-set to work hand-in-glove with Local Care Networks (LCNs) to co-design the CCG's approach to developing Integrated Neighbourhood Networks with frontline staff, patients/service users and carers with lived experience and wider stakeholders.
This approach needs to be driven from the bottom up, supported from the top-down, be scalable across all 9 of Southwark CCG's neighbourhood geographies, and deliver tangible results and culture change on the ground.
This initial phase of support will commence on 29th October 2018 and run through to 31 March 2019, with an option to extend the contract for 3 months, till 30 June 2019.
