description

Mid and South Essex Children's team have identified asthma as a key area to focus on, and as part of this was successful in our bid to be part of the national programme of integrated care models.

This service will enhance the work of the already established asthma team in EPUT to develop integrated care working further, and to embed into business as usual. This will include delivering joint clinics with secondary care, as well as aligning to primary care networks to support the primary care workforce in delivering asthma care. It will also include capacity to work much closer together with universal services and schools to support the school's workforce in supporting children at school.

The Award represents the first two years of funding which have been allocated and guaranteed. National Initiative is a four-year project, any future funding will be reflected in further award notices when allocated and confirmed.

Due to the nature of the process and the well-developed asthma program that it makes both sense and value for money to appropriate this specific funding to EPUT and support the continued integration development.
