Trafford Council is seeking a Service Provider to deliver a new care leaver service providing both accommodation and support for up to 5 young people leaving care from the 1st April 2019. This will be a 2 year contract with the option to extend on annual basis until 31st March 2024.

The Young People

The Service Provider will support young people leaving care between the ages of 16 (i.e. on the school leaving date in the academic year in which they turn 16) up to 21 years with the priority age group being those aged 16-17 years upon admission with medium to high level support needs. Young people supported by this service will present with varying levels of need and may have experienced poor parenting, neglect and/or abuse, including sexual or criminal exploitation. They may have been exposed to substance misuse and may engage in experimental or risk taking behaviour with regard to substances, offending and sexual health; therefore the Service Provider will need to make appropriate links with supporting agencies. The Service Provider will work in close partnership with the Permanence and Transitions team to identify appropriate referrals which will be approved via the Councils Placement Panel.

The Support

We are looking for a Service Provider who can offer a flexible service to support our existing and future demands. We know the needs of young people are ever changing and we need a service that is responsive. The Service Provider will support young people to live semi-independently through 24 hour support with a minimum of 1 staff member at all times and offer flexibility to provide additional support as required dependent on the needs of the young people.

The Accommodation

The accommodation will need to be in Trafford or an area which neighbours the Trafford boundaries. The requirement is for 5 bedrooms for young people. This may be 5 ensuite bedrooms with all other facilities shared or it may be 5 self-contained flats, each having their own front door, bathroom, bedroom, kitchen/living space plus a shared communal area for social activities
