description
NHS Northamptonshire CCG (CCG) wishes to engage with providers in and around Northamptonshire and receive Expressions of Interest from experienced and qualified providers of care homes with nursing for Continuing Health Care (CHC), Discharge to Assess (D2A) and Funded Nursing Care (FNC) services for a future procurement opportunity.
Successful applicants will be required to work collaboratively with the CCG to provide high quality care to patients registered with a Northamptonshire GP who have been assessed as meeting the eligibility criteria for NHS continuing healthcare, NHS Funded Nursing Care or who are on a D2A pathway.
