INTRODUCTION The aim of this event is to provide an outline of the commissioning intentions to potential service providers to gain a wider understanding of the scope of interest and the market's capabilities .The information gathered from this exercise will be used to shape proposals including any potential process and associated service specification. The Council will be holding a supplier engagement day on Friday 16.09.2016. The event will provide an opportunity for The Council to engage with the market and hear from potential suppliers who would like to find out more about the requirement and meet with Commissioners The Council is progressing an ambitious development of its services in line with the Care Act policy drivers. The objective will be to intervene and support people earlier, reduce, defer and delay the need for more intensive support to help people be as independent as possible. BACKGROUND INFORMATION ON DOMICILIARY REABLEMENT AND OUTREACH (1.) Home Assisted Reablement Programme (HARP) is an internal provider service that delivers asocial care reablement model and provides the following: - A planned up to six week intervention to customers in their own home either following discharge from hospital, review of care packages or where there is potential for the customers situation to improve through reablement - A range of support activities that include mobility, meal preparation, support with personal care, sign posting to other agencies for on-going support on discharge(Age UK, church groups bereavement support) and support to carers - A service that is inclusive of people with dementia where there is a reablement need. (2.) Community Intermediate Care Team (CICT) is a multi -disciplinary team, provided by Health that delivers a health care, rehabilitation model for early discharge and admission avoidance. The Council provides a staffing contribution for the social care element of this service. It is only the social care element of CICT that we want to engage with the market on. (3.) Community Based Outreach Team provide support to approx. 22 adults with learning disabilities across three housing schemes in Wolverhampton. The size of the care packages vary from 3 hours to 56 hours per week depending on the individuals needs. The adults with the most complex health and social care needs are supported in one scheme and just over half of the total hours being delivered are used in this scheme. WCC provide a 24 hour call out response to residents.

Additional information: Instructions to Suppliers: The contract or contracts will be awarded on the basis that the Transfer of Undertakings (protection of Employment) Regulations 2006 shall apply to the

existing Council staff team. The Council will be holding a supplier engagement

day on Friday 16.09.2016 from between 2pm and 4pm. The event will provide an

opportunity for The Council to engage with the market and hear from potential

suppliers who would like to find out more about the requirement and meet with
