Tender Opportunity: Social Prescribing in a secondary care setting - 12 month pilot
The Public Health Directorate in partnership with the Health and Care Trust, Acute Trust and the three Worcestershire Clinical Commissioning Groups are working in partnership to develop a model of social prescribing in Secondary Care. This will be adding value to, and using the experience of, the existing developments of social prescribing being rolled out in primary care in Worcestershire.
Funding awarded by Health Education England to the Public Health Directorate has provided provision of a limited number of social prescribing roles for a one-year pilot to test the impact of social prescribing in secondary care settings. Using the existing system of voluntary and community sector providers, this service will bridge the gap between health care professionals and the wider community provision of information and advice.
