description
The City of Wolverhampton Council requires a twelve month programme of very intensive support for 11 to 16 year old looked after young people, their carers and families where the results of conventional interventions have been poor. The provider will set up a multidisciplinary team to work with 15 Wolverhampton looked after young people who are unable to settle in care and move from placement to placement with associated poor outcomes. An intensive, bespoke, approach will be developed with each young person. The contract is potentially three years long and is an outcome based contract. The services will be delivered within the boundaries of Wolverhampton city. The initial appointment will be for a period of twelve months at the end of which the programme implemented by the provider will be evaluated. The Council may decide to extend the contract and there will be provision for two extension periods of up to 12 months each.
Additional information: Information about delivery location: Delivery will need to be where the looked after children are located, which will mostly be within the boundaries of Wolverhampton city but in any case no
further than 20 miles outside.
