Stoke-on-Trent Clinical Commissioning Group (CCG) and Stoke-on-Trent City Council (SoTCC) are looking to jointly commission interim hospital discharge rotas for patients who are medically fit for discharge from a hospital setting, and who require either enablement support or domiciliary care in the community.

The rotas will be commissioned via a block purchased model to create additional care capacity for hospital discharge and increase staff retention in the local social care market for a period of 12 months up to the 28th February 2019. Full details of the requirements are attached to this Notice. Suitable and qualified providers are invited to submit their interest and proposals direct to the CCG and City Council as listed in the other information section.

The CCG and City Council intend to create an approved list of providers that will be approached as and when opportunities arise. the CCG and City Council cannot guarantee any volume of work.

Additional information: Stoke on Trent City Council and Stoke on Trent Clinical Commissioning Group invite providers to bid for the services in line with the attached specification. In order to evaluate the bids we request that each provider submits the following information:

Your Company/organisation details and contact information;
Brief description on how you intend to deliver the service in line with the specification;
Number of rotas you will be putting forward;
Mobilisation plan and timeframes;

Evaluation will be based on how you ensure that you deliver high quality care,your mobilisation plan and CQC rating - scored 0-5.
