Southend-on-Sea Borough Council is inviting expressions of interest from a single (or single consortium) Provider for a Type 2 Diabetes Prevention Programme- Intensive Lifestyle Change course to be completed by up to 1,000 individuals annually, through structured group provision be provided in the community.
The aim of the service is to delay or stop the progression of T2DM in people who have been identified as high risk, by providing an intensive lifestyle change programme. This aim will be achieved by the provision of courses that include:
| • | Pre-diabetes education and the potential development to T2DM |
| • | Nutritional education |
| • | Weight reduction for overweight or obese people (0.5–1 kg / 1–2 lb) a week |
| • | Physical activity |
| • | The Provider will also encourage current smokers to contact local Stop Smoking services. |
The Provider will measure participants at the end of the programme to assess levels of success against clinical and personal goals.
The Provider will ensure delivery of NICE guidelines and:
- Ensure groups meet at least eight times over a period of 9–18 months. Participants should have at least 16 hours of contact time either within a group, on a one-to-one basis or using a mixture of both approaches.
- Offer more intensive support at the start of the programme by delivering core sessions frequently (for example, weekly or fortnightly). Reduce the frequency of sessions over time to encourage more independent lifestyle management.
- Allow time between sessions for participants to make gradual changes to their lifestyle and to reflect on and learn from their experiences.
- Deliver programmes in a range of venues such as workplaces, leisure, community and faith centres and outpatient departments and clinics. Run them at different times, including during evenings and at weekends, to ensure they are as accessible as possible.
- Make use of and refer participants to the Southend Lifestyle Hub who can further signpost and support physical activity, weight management and smoking cessation goals.
The contract term will run for 15 months from 2nd January 2016 – 31st March 2017 with an option to extend for an additional 12 months. The latest expiry date of the contract will be 31st March 2018.
Southend-on-Sea Borough Council welcomes submissions from all organisational types, including the Third Sector.
It is estimated that the contract value will be in the region of £180,000 (for the 15 month + 12 month term).
