Doncaster Metropolitan Borough Council and NHS Doncaster Clinical Commissioning Group invite detailed proposals from experienced Providers to deliver a new care and support service in Doncaster, known as Commissioned Care and Support at Home.
The Contract will commence on Monday 3rd October 2016 and will run for a period of 7 years with the opportunity to extend for a further 3 years (1 X 2 years and 1 X 1 year).
The new service will deliver support to Individuals with varying assessed levels of needs, in order for them to live at home and within their community for as long as it is safe and appropriate to do so. In all cases, the focus is on helping people to optimise their independence through the provision of personal care, support with daily living, essential domestic tasks and the promotion of wellbeing. The focus is on achieving the agreed outcomes, as defined by the Individual. Person centred planning is robustly embedded and puts the Individual and their Carers at the forefront of all decision making.
The contract opportunity has been split into seven (7) Lots as follows:
ï Lots 1 to 6 - Strategic Lead Providers
The Borough will be split into 6 geographical areas, with Strategic Lead Providers able to bid for up to two areas (a minimum of 4 Strategic Lead Providers are required to operate within the borough). ï Lot 7 - Additional Support Providers.
Contracts shall be awarded to tenderers that pass the selection process and achieve the minimum quality score of 60% and will have the opportunity to deliver services borough wide.
Tenderers should note a tender information event to provide information on the procurement process will take place on the 9th of June 2016. There will be two sessions, one in the morning and an identical session in the afternoon. A lunch will be provided for all attendees to act as a cross over between sessions.
The sessions are as follows:
10:30 - 12:00 AM session
12:00 - 13:00 Lunch
13:00 - 14:30 PM session
