The Acute Frailty Programme deliverables are: • The programme aims to redesign the first 72 hours of the system's frailty service from the first call from the patient or their carer. • The aims are to avoid admission where possible, if admission is needed, to ensure that the patient is seen in a same day emergency care environment or at most a 72 hour frailty unit to avoid deconditioning associated with long stays in hospital.
