description

North Tyneside Council have created an integrated model of services for children and young people aged 0-19 years, which comprises four locality based integrated teams, delivering universal and targeted services to children, young people and families.

These teams bring together a range of practitioners into an integrated prevention and early integration model of service delivery with a clear aim to promote social, emotional and mental wellbeing, building resilient and healthy children, young people and families, preventing family breakdown and ensuring that children and young people are ready for school and life.

This contract covers the children public health element of the integrated teams. These have traditionally been known as Health Visiting and School Nurse Services.
