- oprovide infrequent 'step up' referrals to avoid hospital admission with a subsequent prescription of care being undertaken in the person's own home
- oprovide infrequent 'end of life' care and support packages to enable an individual to die in their own home
- oassist in the development of a responsive service referral and assessment process
- oassist with the development of the local technology in care offer
- outilise the functionality of electronic care management system to record goals/outcomes
- oevaluate the success of this pilot including the collection and analysis of customer feedback
