NHS NEL CCG (BHR Region) are looking for an organisation to work in partnership and collaboration with to develop training and digital systems to support the Domiciliary/Home Care workforce, to monitor the health of vulnerable older people in their own homes.
The benefits of the service will be:
| • | Reducing ED and Non Elective Admissions |
| • | Providing care closer to home |
| • | Supporting expert and enhanced home and domiciliary care approaches |
| • | Supporting primary care to monitor vulnerable patients in the community |
The successful bidder will work across BHR (potentially with 2 targeted boroughs) to develop:
| • | A training programme and on-going support for domiciliary care staff |
| • | Provide digital apparatus to monitor patients |
| • | Have a research partner to evaluate the impact of the pilot |
| • | BHR are looking for a partner who could also match or identify other funding sources to support the overall delivery of the pilot |
The pilot will run for 12 months from the point of commencement, supporting approximately 350-400 patients. BHR are looking for the pilot to commence by 15st December 2021, and the estimated contract value per annum non recurrent will be £100,000 (including Project Management, research and care hours backfill).
