description

Working together under co-commissioning arrangements, NHS England and the Berkshire West CCGs procured GP led primary medical care services for 3 GP Practices in the Reading and Wokingham areas of Berkshire. Providers needed to demonstrate experience of implementing innovative change, resulting in real benefits for patients, and to provide services in such a way as to support delivery of the NHS 5 Year Forward View and the Berkshire West Primary Care Strategy.

The three practices are:

Lot 1 Circuit Lane Surgery, Reading Lot 2 Priory Avenue Surgery, Reading Lot 3 Shinfield Medical Practice, Wokingham

The Berkshire West Primary Care Strategy is based on the following five strategic objectives. The Provider is expected to ensure these are reflected in the services that they provide.

  • oAddressing current pressures and creating a sustainable primary care sector
  • oInterfacing in new ways with specialisms historically provided in secondary care to manage increasingly complex chronic disease in a community setting
  • oManaging the health of a population in partnership with others. Acting as accountable clinicians for the Over 75s and other high risk patients and co-ordinating an increasingly complex team of people working in primary, community and social care to support patients at home.
  • oUsing new approaches and technologies to improve access and patient experience, ensuring that the needs of patients requiring urgent primary care are met
  • oMaking effective referrals to other services when patients will most benefit.

The contracts are for the following terms:

Circuit Lane Surgery 10 years

Priory Avenue Surgery 10 years

Shinfield Medical Practice 3 years, with the option to extend for up to two further years reserved.
