Stop Smoking Service and NHS Health Checks

The purpose of the Tender is to identify suitably qualified organisations to provide high-quality Stop Smoking Services and NHS Health Checks across Herefordshire.

This Any Qualified Provider (AQP) procurement offer is designed to support the development of Stop Smoking Services and NHS Health Checks in Herefordshire. It will provide funding to assist with the identification and proactive management of patients with modifiable risk factors for disease.

Services covered by this AQP procurement have been separated into two Lots:

LOT 1 – Stop Smoking Behavioural Support (general and pregnant smokers)

LOT 2 – NHS Health Checks

LOT 1:

The purpose of the Stop Smoking Service is to reduce the number of smokers by encouraging and supporting smokers to quit the habit and adopt a healthier smoke free lifestyle and encourage provision of an enhanced service to support pregnant smokers to quit.

LOT 2:

The aim of NHS Health Checks is to identify individual risk of vascular disease (heart disease, stroke, diabetes, kidney disease and certain types of dementia) in the local population aged 40-74, for those not already diagnosed and provide appropriate advice and interventions to reduce or mange that risk.

The contract will run up until the 31st March 2018. Further Tender applications will be accepted up until twelve months from the end of the contract, offering further prospective Tenderers the opportunity to provide a service for the remainder of the contract duration. Please note this is for a PBR (payment by results) contract with no guaranteed contract volumes.

Additional information: This procurement will operate in the same way as a Dynamic Purchasing System (DPS) and will remain open to new suitably qualified providers until 31st March 2017. The closing date specified in this notice represents the closing date of the first round.
