Home from Hospital Service will provide a proactive low level support service for adult residents of County Durham and Darlington Borough, to facilitate a smooth transition home from hospital or, on referral from either a County Durham Intermediate Care Plus team or Darlington RIACT Plus, to provide low level support to an adult who has recently been discharged from hospital.
