Contract for the Provision of GP Services relating to NHS Health Checks and Sexual Health Specialist Services

Additional information: This is a voluntary notice in relation to a contract for community health services which fall within Schedule 3 of the Public Contracts Regulations 2015.

All registered GP practices within the Borough are invited to join the Contract and Practice Managers will be notified by email.

PLEASE NOTE: Not all of the services will be available for provision by all GP practices. Services will be based on need in the area and provision will be confirmed in a covering letter issued with the Contract. Any areas not reflected within the Contract will be annotated "Not Applicable".
