The Service will provide community-based support for people recovering from mental ill-health. It will play a key role in linking them with existing community networks and opportunities. The Service will be available to people receiving mental health services under the Care Act Eligibility Guidance The Service will operate a model based on the principles of recovery and rehabilitation, aimed at supporting people recovering from mental ill-health to move through the service and beyond mere survival and existence, encouraging them to progress and gain or retain employment, carry out activities and develop relationships that give their lives purpose and meaning, with a focus on promoting choice, control, independent living, social inclusion and wellbeing. The Service will work with care co-ordinators to facilitate planned interventions which will typically mean the provision of face-to-face, 1:1 practical, personal and social support both in the community and in people's homes. It will include, but will not be limited to one or more of the following: · Support with practical tasks, with the emphasis on encouraging people to retain or regain and carry out daily living skills with increasing independence · Support in managing day-to-day finances · Support in accessing mainstream services and resources including involvement in leisure and recreation interests and pursuits on a regular basis. Knowledge of community resources both specialist and mainstream is therefore essential. · Support in maintaining employment or encouragement in re-entering employment, taking part in daytime activities or volunteering that will promote employment readiness. · Support for people to have a central role in the development, implementation and review of their support plans and to adapt support to meet the sometimes complex and changing needs of people. · Support for people to identify and engage in healthy living and wellbeing practices. · Appropriate liaison with regard to any improvement or deterioration in any aspect of a person's life, including early signs of relapse, with care co-ordinators and appropriate others. · Support access to and the use of public transport and to travel training schemes where appropriate
