description
The Carers Discharge scheme is a short term initiative funded from the ASC Discharge Grant to facilitate hospital discharges up until the end of March 2024. The scheme will make one off payments of up to £500 to support family/informal carers who are providing support to enable a cared for person to be safely discharged from hospital and for them to be supported at home.
