Stroke Support Services Haringey CCG

Haringey Clinical Commissioning Group (HCCG) and London Borough of Haringey (LBH) wish to commission a high quality support services for stroke survivors and their carers in the borough. The service will aim to promote independence and enhance the health and wellbeing of individuals who have been affected by a stroke and their carers so that they can participate as full citizens in the community.

The Service will be focused on supporting individuals who have been affected by a stroke and their carers access everyday activities such as leisure, education, training and work (paid and unpaid). This is especially important in preventing more serious psychological consequences of stroke and in fostering self-reliance and healthy living after stroke.

The Service will support people and their carers navigate the services and support available across the system and work in close partnership with a wide a range of statutory and non-statutory agencies in order to ensure improved outcomes and user experience.

The contract term will be for three years with the proposed commencement date from 01 April 2017. The services are being procured by Haringey Clinical Commissioning Group (the CCG) and the NHS standard contract terms shall apply.

The CCG is looking to appoint a suitably qualified and experienced Service Provider who can demonstrate an in-depth understanding of the local demographics and needs of stroke survivors and their carers, innovative and evidence-based approaches to meeting these needs and with a track record of involving people and carers in service design and delivery as well as establishing and fostering close working relationships with a wide range of partners.
