The Commissioner (Coventry City Council and Coventry & Rugby Clinical Commissioning Group) is looking to contract with a provider for the delivery of long-term home support, specifically for the Wyken, Binley & Willenhall and Lower Stoke wards of the City (Cluster 6). This will be for Adults and Children that meet national eligibility criteria for social care and Adults eligible for non-complex health funded Continuing Health Care at home. The service will provide personalised care and support to people in their own homes to enable individuals to live independently. The service will also assist carers/parents to maintain their caring role.
