The five CCGs in North Central London have committed to commissioning health services based on outcomes, with an initial focus on three population groups:
| 1. | People with diabetes; |
| 2. | Older people with frailty |
| 3. | People with severe mental illness. |
This contract is for the finalisation, validation, piloting and implementation of survey instruments that will measure pre-specified patient level outcomes in the three value-based commissioning population groups in North Central London.
The anticipated outcomes of the proposed service are as follows
