Greater Huddersfield Clinical Commissioning Group (GHCCG) and North Kirklees Clinical Commissioning Group (NKCCG) have embarked upon a collaborative project to jointly commission Care Closer to Home Health Care services across Kirklees.
The commissioning of community healthcare services across Kirklees has enabled services to be designed to enable the development of outcome based service specifications allowing greater transparency of quality of standards and performance management against the targets.
The Care Closer to Home Strategic case set out the vision for the development of integrated services across Greater Huddersfield, North Kirklees and combined across Kirklees as a whole, with improving patient’s experience, supporting self-management and integrating health and social care being the main priority for the CCGs and their key partner commissioners.
Work has been undertaken through the competitive dialogue process to redesign community health services across the entire system this enabling; excellent quality of services and cost effective care delivered within peoples own homes where possible to avoid expensive and unsettling acute admissions to hospitals, nursing homes and other inpatient facilities when safe to do so.
The key underpinning element is to ensure joined up services for the benefits of the patients, supporting the National direction of travel which is driving integration both in the commissioning provision and of health and social care, the introduction of the Better Care fund being an enabler to support the work.
The successful solution will ensure that primary, community and social care can provide the right care in the right place, first time by staff who have the competencies and skills to meet the needs of patients / clients which complement and work together with acute services.
People will receive care that is more timely and organised to meet their specific needs, the services will be co-ordinated across providers where necessary ensuring care is coordinated and seamless as one coherent package with a focus on recovery and promotion of independence.
In order to achieve the vision, the CCGs are implementing a Prime/ Lead provider model, with the service provider working across Greater Huddersfield and North Kirklees CCGs in collaboration with other providers to deliver the required outcomes for Care Closer to Home.
The contract is a five year contract with the option to extend for a further two years
