Worcestershire Community Dietetics

The Procurement is for a comprehensive community based clinical dietetic service to Worcestershire. The service will aim to reduce malnutrition in Worcestershire through a focus of support to defined patient groups. This will be achieved by identifying those at risk of being malnourished, establishing appropriate care plans, education of patients, carers and health professionals and maintaining on-going support where appropriate. Direct support and expertise from a dietitian is to be available where this is required for particular identified patients with complex nutritional requirements.

This service is designed to be available to people registered with a GP in Worcestershire and will support patients across all age groups with a variety of conditions to make long-term lifestyle changes, including diet and behaviour, in order to increase quality of life and longevity. The service provider will be required to work collaboratively with existing providers of inpatient and outpatient dietetic services in Worcestershire as part of an integrated, evidence based approach to patient care. The service provider will be required to evidence how integration has been achieved.

Contract start date will be subject to the wining bidders mobilisaton plan.

The Commissioners reserve the right to not proceed with bids above the indicated budget.
