NHS Bromley CCG is seeking to provide a clinical pharmacy advice service working at individual practice and patient levels. The roles will be based within the GP practices of one of the Integrated Care Networks (ICNs) covering a population of over 100,000 patients and 15 GP practices. It is anticipated that 3 pharmacists will be required to cover this population and the pilot will last for 18months. NHS Bromley CCG wants to commission local community services in line with the Bromley Out-of-Hospital Transformation Programme. Healthcare services cannot operate in isolation and must be delivered in line with key principles of integration, including the implementation of three ICNs across the borough to deliver better integrated working between primary, community, and all other health and social care services. By integrating care pathways, patients will have better access to the right care, closer to home, from the most appropriately skilled professional and at the time of need. Patients will be at the centre of care planning, commissioning, delivery & support, resulting in a greater focus on ill-health prevention, delivering proactive & responsive care when it is needed and helping to maintain patients’ independence for as long as possible.
Additional information: Please note that the purpose of this notice is to inform potential providers of NHS Bromley CCG’s intention to provide a clinical pharmacy service based within GP practices within one of our developing Integrated Care Networks.
At this stage, potential providers are only asked to submit an Expression of Interest.
