Provide support to establish a Transforming Care Partnership (TCP) to implement "Building the right support - A national plan to develop community services and close inpatient facilities" for people with a learning disability and/or autism who display behaviours that challenges, including those with a mental health condition. This plan will cover the LA and health economies of Telford & Wrekin and Shropshire.
