Facilitated by Halton Borough Council for and on behalf of NHS Halton Clinical Commissioning Group (NHS HCCG). NHS Halton CCG are seeking a provider from suitably qualified and experienced organisations to provide high-quality structured education for adults with Type 2 Diabetes, as part of increasing the capability and capacity of the healthcare system as a whole to meet the needs of growing numbers of people with diabetes. The objectives of the service are to: - Provide structured education to adults with Type 2 Diabetes. Individuals will be able to access services which are evidence based and of the best quality delivered by competent professionals. - Provide accessible services as close to patients's home or work as possible. - Promote partnership working and a shared care approach between providers, so patients experience appropriate care, seamlessly, and in a timely manner. - Ensure that individuals receive care that is integrated and consistent across organisational and professional boundaries, with robust integration and cooperation with other providers. The service will provide modules of education for patients with Type 2 Diabetes that meet the criteria set in NICE clinical guidlines on Type 2 Diabetes. TUPE arrangements do not apply. Potential bidders should note that there has been no provision offered to newly diagnosed patients with effect from 1.7.15 and the new contract holder will be expected to offer education to this group of patients within the first 3 months of the new contract; in addition to the normal patient referrals, therefore managing an acquired waiting list.
