NHS Leeds and NHS Bradford Clinical Commissioning Groups are requesting quotations from interested providers for the provision of a Special Allocation Service (SAS) Safe Haven Service.
The purpose of the SAS is
| - | to ensure the provision of Primary Medical Services for patients who have been subject to immediate removal from a practice list of a primary medical services contractor because of an act or threat of violence. (Age 16+) |
| - | to provide a stable environment for the patient to receive continued health care. |
| - | to address any underlying causes of aggressive behaviour by promoting a continuing understanding of the NHS health and social care system to encourage the patient to use the service. |
| - | To return patients to mainstream primary medical care services when it is clinically appropriate and safe to do so. |
There are currently 100 patients registered across the area. These figures fluctuate due to the nature of the patient cohort and the ultimate intention to return patients to mainstream general practice where appropriate.
In our new service we are looking for:
| - | A Greater say in where services are located, based on areas of highest need, and tackling health inequalities |
| - | A new way of working which prioritises population health management, personalisation of care, and rewards delivering patient outcomes |
| - | Flexibility of service delivery for providers to work with partners in primary care, commissioning, secondary care and non medical services to innovate and improve quality. |
| - | Shared responsibility between provider and commissioner, where providers have confidence to plan and innovate based on secured income, and adjustments in pathways or ways of working can be adopted rapidly in the light of changing circumstances. |
The CCG Partnership is interested in a lead/single provider model to provide an integrated and outcome based service across the Leeds and Bradford Care Systems.
The proposed contract will be for a period of 2 years starting on 1 July 2021 and may be extended for a further period up to 2 years.
