This is a Request for Information (RFI) published on behalf of NHS Southern Derbyshire Clinical Commissioning Group (CCG).
NHS Southern Derbyshire CCG is representing the four CCGs in Derbyshire including, NHS Erewash CCG, NHS Hardwick, NHS North Derbyshire and NHS Southern Derbyshire CCG.
The purpose of this RFI is to gauge market interest for the provision of Continuing Health Care Services.
CCGs have a statutory duty to meet the requirements of the Health and Social Care Act (2012) including the provision of NHS Continuing Healthcare and NHS Funded Nursing Care. This is undertaken by assessing, reviewing and case managing patients who are their responsibility where they have been identified as likely to be eligible for NHS Continuing Healthcare, or a joint funded package of care or are placed in a nursing home and are eligible for receipt of Funded Nursing Care.
The CCGs within Derbyshire are collaborating on the commissioning of Continuing Healthcare Services, however each individual CCG remains statutorily accountable for the provision of all elements of NHS Continuing Healthcare for its registered population. The service specification will cover the services provided for the aforementioned CCGs.
The specification is currently being developed with the knowledge of the NHS England 5 year Forward View and in the context of the strategic vision of the CCGs within Derbyshire.
The aim is to create a seamless health and social care service, which is patient focused, promotes independence, is delivered as close to home as possible, and to achieve a focus upon ‘place-based systems of care’ working in collaboration and partnership across organisations.
With this RFI the CCGs are seeking to learn the level of possible interest from organisations in relation to providing a service for Derbyshire, including innovative delivery models fit for the future. The same information will be gathered from different organisations to inform the CCGs service model and future procurement plan.
