Evaluation of a Complex Care Case Management Pilot Service

Camden CCG wishes to engage with a suitably qualified provider to undertake an evaluation of a Pilot of a Complex Care Case Management Service. The Pilot service provision is for two years throughout the period 1 April 2014 to 31 March 2016. The contract value for the evaluation of the pilot service should be between £20,000 and £25,000 excluding VAT.

The chosen provider is expected to commence the work in the week commencing 3rd August 2015 and finish the work on or before 12th October 2015.

• Your response should consist of no more than 5000 words;

• Expected delivery / start / finish date, and a project time table;

• Total price excluding VAT, including all costs on the template provided (Annex 6)

• Confirmation of acceptance of the terms and conditions of contract (Annex 4);

• Conflict of Interest Declaration (Annex 5).
