Medway Clinical Commissioning Group (CCG) will be commencing formal procurement for a number of service provisions this year. Commissioners are looking to get early engagement with providers and are running Market Engagement events.

  1. Dietetics & Nutrition

Service aims are to:

Improve the health of those suffering from nutrition related diseases.

Provide specialist assessment, clinical diagnosis and treatment of patients who have a nutritional related disorder.

Proactive prevention of malnutrition in the community.

Train healthcare professionals in the screening, prevention and treatment of malnutrition and nutrition related disease.

  1. Anti-Coagulation; Arrhythmia

To provide a primary care Arrhythmia Service for Medway residents. The Service will highlight the need to identify patients with arrhythmia in the community via education and awareness raising in social care, nursing, GP and other community services and where necessary providing adequate assessment, treatment planning and education. The Service will also provide direct advice to GP practices in the optimal treatment of patients with Arrhythmia.

Anti-coagulation

The Service aim is to promote and provide a high quality, effective, accessible, timely, responsive, non-discriminatory cost effective community based anticoagulation therapy service that will address the health needs of the population at flexible sites, through a flexible model of delivery what significantly contributes both to improving the well-being of the population and to reducing inequalities.

  1. Tissue Viability & Wound Therapy; Podiatry; Lymphoedema

The Tissue Viability and Wound Therapy service is for all patients registered with GPs in NHS Medway who have an open wound, or is at risk of skin breakdown because of trauma or disease processes.

The Podiatry service provides an integrated holistic multi-disciplinary service which aims to improve the medical conditions affecting the foot and lower limb for all patients registered with GPs in Medway. Provision includes community and/or community hospital in-reach Podiatry services, either as autonomous practitioners or as part of multi-disciplinary teams to include clinical care, training, research and health promotion for medical problems affecting the foot and lower limb

The Community Lymphoedema Service aims to provide a comprehensive, community based service for all patients with Lymphoedema, regardless of cause, with access to appropriate treatment nearer to their homes, hence avoiding hospital treatments and improving the patient’s quality of life.
