<b>Introduction</b> LMH and LMH groups are looking to potentially commission services for a health and wellbeing service this is to be a 2 years with the option to extend for a further 12 months. Subject to requirement and funding: The purpose of this requirement is to work across LMH neighbourhoods, actively engaging residents to improve their health and wellbeing through sport, physical exercise and positive activities to promote behavioural change. The successful organisation will work with partners to develop projects for promotion of the service i.e. · Healthier Lifestyles · Address Inequalities · Recognise & Address low self esteem ( in relation to Social/economic factors) · Develop programmes with partners · Improve and prevent ill health With experience of working with communities as part of a team focussed on health and wellbeing initiatives, and delivery projects that address health inequalities associated with deprived neighbourhoods, bringing new innovative ideas support and good communication to the service. For this service knowledge of issues and impacts in relation to health and wellbeing across city neighbourhoods and a understanding priories in this field, bringing an integrated approach to the working to improve health in a range of areas by developing a calendar of events and a community strategy, for example · Risk of isolation · Substance abuse/Mental Health · Reduce child obesity levels · Addressing Nutrition (on limited budgets) · Financial hardship · Youth engagement · Behaviour
