Bespoke Adult Social Care Review Project

Swindon Borough Council invites expressions of interest from suitably qualified and experienced service providers for the provision of undertaking a number of overdue Adult Social Care reviews/re-assessments . This is likely to increase the capacity in front line operational teams. We expect that by undertaking these reviews a percentage of savings will be unlocked within our funded care and direct payment cohort and that quality and performance will be improved within Adult Social Care The managed team of experienced social workers will undertake a person-centered approach. The social workers will, throughout their intervention, ensure that the person's voice and wishes are heard and responded to. This approach will ensure that the person is supported to identify and recognize their strengths and abilities and how to use them, with the help of practitioners, to meet their desired outcomes.

The contract term will be for 6 months as stated in the ITT documents. . The estimated contract spend is £500,000
