Herefordshire Council and Herefordshire Clinical Commissioning Group (CCG) acknowledge the significant contribution of carers and are committed to supporting them to fulfil their caring role. The Council and CCG are equally committed to working with carers and partner agencies to commission preventative services that meet carer's needs and promote their health and wellbeing. The Council and CCG wish to commission an innovative, person - centred Carers Health and Wellbeing Service. Provider(s) will deliver a proactive, high quality support service that is carer focused, promotes independence and provides personal choice.
