NHS Bristol, North Somerset and South Gloucestershire Clinical Commissioning Group (the Commissioner) seek to appoint provider(s) for support for General Practices to develop active signposting in the Bristol, North Somerset and South Gloucestershire area.
The services are split into two lots:
- Lot 1 - Flexible individual practice support to upskill and support the practice reception team to actively signpost patients
- Lot 2 - Structured programme of process change support and facilitation to develop approach to active signposting, including group working with other local practices
The contract period will be for 12 months from commencement.
The budget across both lots is approximately £100,000 exclusive of VAT (where applicable). GP practices will make their own selection of the two lots based on the most suitable approach for their service model. The individual Lots do not have maximum values, with the ultimate value of each Lot being determined through usage.
