Recommissioning of the Home Improvement Agency Contract. This is a key preventative service that reduces hospital admissions and supports people to remain at home for longer, avoiding costly care placements. The service will also work to maximise hospital discharges by providing advice & handy man services in relation to adaptations within service users properties.
The contractor will provide:
Case work & technical advice
Adaptations advice
Handy Person Service (for minor works)
Home from Hospital case work service (working to help maximise hospital discharges)
General information & advice to service users
The HIA has been commissioned to deliver the following outcomes:
To help residents of Bristol to remain in their own homes/ in an alternative home that better meets their needs To help residents maximise their independence and improve their wellbeing
That Bristol residents using the service feel safer, warmer, and healthier
To meet the anticipated need for home improvement and adaptation services
To avoid medical and/or social care which is the result of unsuitable or unsafe housing
To increase the number and success of hospital discharge and to prevent readmission

Helps Bristol citizens remain in their own homes/alternative accommodation that meets their needs.
Cut costs by reducing re-admissions to hospitals.
Bristol residents maximise independence and have been proven to feel safer, warmer and happier.
