The London borough of Newham invites tender for the completion of the scheduled reviews and appropriate outcome based care and support plans. The review will undertake complex and non-complex reviews including Care and Support plans, Continuing Health Care, Mental Capacity Act, Decision Screening Tool and/or carers assessments - as required.
