Providing a primary care service to holistically and proactively manage the physical and psychosocial needs of SMI patients (as identified on GP practice registers) through audit, identification, screening, signposting, co-ordination and follow up. The service provider would be expected in addition to provide interventions such as lifestyle behaviour change. The contract will be an outcomes based model with a 70/30 split of contract payments – with 30% linked to delivery of agreed outcomes.
NHS Camden CCG is keen to test the potential provider market for delivering a service which improves the Physical and Psychosocial Outcomes for patients with a Serious Mental Illness (SMI) in primary care
The chosen provider should be able to do the following:
| • | Access the GP SMI registers of all 36 Camden GP practices |
| • | Support a Camden wide practice based audit process to case find and improve coding of patients over the age of 18 who should be on the practice SMI register. |
| • | Recruit and employ a nursing team (with a minimum of level 2 smoking cessation training) to be a dedicated resource to primary care to undertake holistic physical and psychosocial screening, signposting, interventions and follow up |
| • | Co-ordinate the process of ensuring the above team are able to invite patients over 18 with an SMI for a holistic assessment to identify and manage physical, mental and social risk factors – to ensure early intervention and prevention of long term conditions. |
| • | Facilitate the smooth process of all Camden practices engaging with the Team around the Practice (TAP) to develop a practice specific training needs analysis and the on-going process of ensuring practice members engage in training. |
| • | Provide regular updates and performance data to Camden CCG as the lead commissioner |
| • | Deliver an outcomes based contract |
The provider organisation must have the capacity, skills and expertise to manage and deliver this service providing universal coverage across Camden, for a 2.5 year initial period. The provider must be CQC registered and have in place appropriate governance systems, including the ability and agreement to access GP registers of patients in all Camden GP practices.
Contract Value - £273,000 - £390,000 *indicative
Providers must be able to mobilise quickly with capacity in place to commence the service by no later than end September 2015.

Additional information: Please note that this is a market testing exercise to inform the CCG’s decision making process. This does not commit the CCG to a carry out a procurement process.
If a procurement exercise is subsequently undertaken, a shortlist of economic operators will be compiled and invited to submit bids for the project.
