Medway Council are seeking assistance with the transformation of their approach to delivering Adult Social Care. Currently the approach undertaken follows a traditional Short term assessment, enablement, long-term assessment, long-term support plan, care package approach. There is an increasing demand on Medway Council's Adult Social Care resources. Medway Council are looking to adopt an approach that will increase the use of family and community resources and change the approach to service delivery from a care package by default to a care package as last resort as a solution. This approach is looking to deliver on the promises of personalisation, instilling a cultural change within Adult Social Care that leads to the use of long-term care packages becoming the approach of last resort rather than the default option. Although Medway Council have a vision of how they want to deliver Adult Social Care in the future, they require assistance in the design and implementation of this approach and seek quotations from appropriately skilled and experienced consultancy companies to aid them in the design and implementation.
