description

Commissioning Intention

The CCG are seeking to commission the programme development, management and delivery of its Peer Health Champion Programme.

Background:

Building on the success of the CCG migrant peer health training programme initiated in 2014-15, the CCG will look to further develop and enhance this model into a structured peer training and community health education programme.

Aim

The programme seeks to improve the health awareness and literacy of diverse and vulnerable community groups to enable them to make informed choices concerning their health and well-being. Through reinforcement of key health messages, it is anticipated, that in the long-term, the programme will lead to positive behaviour change within the priority areas identified by the CCG.

Objectives

The Peer Health Champion Programme will seek to:

•Provide access to a series of structured health education, learning and development sessions for organisations supporting diverse communities across the CCG footprint.

•Equip each organisation to have the requisite skills and knowledge within areas of priority identified by the CCG.

•Empower each organisation participating in the programme, to attend peer training and to commit to cascading their learning of key health messages to the local communities they engage with; •Encourage the development of culturally sensitive and language appropriate resources and materials.

•Engage health professionals in the development and delivery of a structured training programme within their area of specialism.

•Enable the CCG to benefit from having access to a network of community organisations for engaging, informing and instigating behaviour change within hard to reach and vulnerable groups. •Deliver an annual winter resilience campaign utilising existing community and voluntary sector trained peer champions
