Extra Care - personalised Care and Wellbeing service for people aged over 55

provision of a personalised Care and Wellbeing service for people aged 55 and over with physical and/or sensory disabilities and/or learning disabilities and/or mental health needs (including memory and cognition) who have been assessed as meeting the National Eligibility Criteria (Care Act, 2014) and who are either ordinarily resident within the

London Borough of Hillingdon or are the responsibility of Hillingdon London Borough

Council (the 'Council') for supporting their social care needs.

Additional information:

The contract will be for provision of a personalised Care and Wellbeing service for people aged 55 and over with physical and/or sensory disabilities and/or learning disabilities and/or mental health needs (including memory and cognition) who have been assessed as meeting the National Eligibility Criteria (Care Act, 2014) and who are either ordinarily resident within the London Borough of Hillingdon or are the responsibility of Hillingdon London Borough Council (the 'Council') for supporting their social care needs.

The Contract will be for an initial term of 5 (five) years extendable at Hillingdon's sole

option for a period of 2 + 1 (two + one) years, giving a total of 8 (eight) years (the

Term)
